Salary: 72.5K/yr ($35/hr) to 93.6K ($45/hr)
Job Summary: The program manager will oversee the HealthierHere (THRIVE) team. The manager will also handle budget management and team meetings. Additionally, the manager is expected to use Zero-balance budgeting and submit reports and ledgers for invoicing. Position is 60% spent on managing the Thrive grant and 40% on clinical supervision with the Director of Mental Health Services.

Duties and Responsibilities:
- The manager will review documentation by assigned case managers in their designated client management system (CMS) regularly
- Manager will be available for regular case staffing, problem-solving with their assigned case managers, and providing regular feedback for guidance to support documentation and client interactions.
- Manager will provide mentorship and support to assist case managers in: reviewing intake information; collecting client consent to enroll clients into the program; Conducting Health Related Social Needs (HRSN)assessments; Developing and managing client goals; Providing coaching, health education, resource navigation, and referral support to individuals and households; Completing case closure and ensuring referrals to long term services is established as needed
- The Manager will communicate any noted changes in case management or documentation related to the care of the individual or household to the case manager as soon as possible
- The Manager will coordinate case reviews and documentation of case managers’ caseloads with the Director of Behavioral Health on a bi-weekly basis.
- The Manager will ensure that case managers have appropriate training and referral protocols for potential crisis situations. This includes, but is not limited to, mandatory reporting, danger to self or others, and medical crises.
- The manager must notify the Community Hub Manager and their case managers whenever they are out sick or on PTO. A backup Manager from the agency must be identified and communicated to the Community Hub Manager when out of office or on leave.
- Manager, we will continue to make note of ongoing issues and successes related to case manager performance.
- Manager will share any training / TA leads with the Community Hub Manager
- Manager will attend all scheduled Manager meetings and trainings, including regular 1: 1 check-ins with the healthier here Community hub.
- Manager will review caseloads with case managers on a biweekly basis
- Manager will share case manager schedules with the community hub manager each week to facilitate assignment of referrals to the staff. If changes to the schedule due to a Case Manager being out or sick or on leave unexpectedly, the Manager should notify the Community Hub Manager as soon as possible.

- Coordinate and collaborate with other prevention programs and services within Seattle/King County to better serve targeted populations and the community as a whole
- Develop relationships with community leaders and serve as the organization's liaison with various constituents  
- Works well under general supervision
- A degree of creativity, initiative, and latitude is required
- This position will require working some weekends, evenings, and holidays
- Other duties as assigned
Professional Qualifications:
- MSW preferred, or related degree
- Budget management experience
- Excellent written and verbal communication skills and ability to present to diverse audiences, specifically racially, ethnically, gender diverse, and socioeconomically diverse communities.
